What Do I Need to Know About Forms?
Completing paperwork is a necessary part of receiving counseling or treatment at NorthStar Regional. Most of the information we need before your first visit can be completed online, or you can complete your paperwork at the time of your first visit. If you plan on completing necessary forms at our office, please arrive 20 or more minutes before your appointment time.
